Ok, I've a tough one:

I come across a lot of stubborn sciatica. By this I mean that the person continues to have pain when structurally/orthopedically/osteopathically there is no real reason. What I find is nothing, from moxa to cups seems to help. The pain can range from pins and needles to cramps. Addressing the underlying pattern doesn't seem to help. Most often, the pains persist most in the buttock and the calf.
My only clue is that most of the people I come across in this condition have taken injections of Voltaren and Muscaril (anti-imflamatory/pain killer) in the buttock. But more specifically, they have taken these substances taken straight from the fridge, given that "KEEP REFRIGERATED" is written in large letters on the box. Any ideas any one?

Sciatica, usually related to ruptured or otherwise damaged disc, or spinal stenosis, may be treated effectively by acupuncture, despite the irreversible orthopedic/anatomical damage. Such cases, in order to be effectively treated, need very good needle technique + other modalities sometimes.
Needle technique must be a vigorous dispersion and very accurate point location. Cold patterns must be treated with extensive moxa heating, and blood stasis patterns with recurrent blood cupping. In very serious or stubborn cases, direct moxa scarring may be very helpful. Electro-acupuncture can also be very effective for the pain during the regular sessions.

Have you tried TAT or EFT? These are energetic approaches that target the
root rather than dealing with the branch. Both manuals are available to
download free of charge at www.emofree.com for EFT and www.TATLife.com for
TAT.

Hi there, Have you checked the outlets of the Sciatic nerve in the back of
the abdo?
I have written quite a bit in the forum on the Psoas muscle.
If it is tight (or the Iliacus) it will trap the origin of the Sciatic N
poss at L4/5, L5/S1, S1 or S2.
If these show any tenderness on palpation into the abdo down the St
channel(ish) from level of Ren 12 to St 28, then pin these depth of about 3
to 4 MM.
This is like trigger points and will loosen up the Psoas over three or four
treatments.
I always start there and check this area first.
My (25year) experience says start at the origin of a Nerve.
I think 85% of all back pain comes from a spasmed psoas.

Check the previous posts on Psoas or Sciatica on Shmuels site and read up on
what I've said.

To Shmuel...did these get hacked or are they still available.

Clear out the 8 extraordinary vessels. Do SI 3 and UB 62. Then check it
out.

Next, find the disc that is causing the problem and needle the Du point (or
make one up) and the Huatuojiaji at the same level and do the inner UB
points so you have 5 needles all at the same level.

Try that and see what you get.

Hey Guys,
Thanks for all the feed-back.
I have tried clearing the Du, the Huatuojiaji, cupping, bleeding, crying, but this is the reason why I posed the question. Notwithstanding all of this, the pain, especially in the calf muscle just doesn't want to go. I should have been a little more clear: three out of four of the cases of this kind present no pain in the back. The osteopath who sends me these people (a serious dude) assures me that the psoas and piriformis are all happy and dandy. I think one of the reasons why he sends me these people is precisely because he can't work it out either.
I've thought about using an eight pound hammer, but I can't find any references in the Nei Jing.
Keep the ideas coming, folks, these people are hurting.

Where is the calf pain?

If you were to divide the calf into two parts, inner and outer; and divide it further into three more parts inner and three more parts outer... then each part would be upper (inner and outer) near UB 56, middle (inner and outer) near UB 57, and lower (inner and outer) a bit lower... thus giving you 6 parts of the gastrocnemeus. (Talk about a run on sentence!)

Palpate each division and write back on which leg and which of the divisions: upper, lower, middle, inner, outer of which leg is the most tender or painful.

When you tell me and I confirm it, I'll write back and give you 4 points to do on the opposite side of the body.

If the pain is unremitting despite everything you have done, I would suggest some root emotional work such as TAT or EFT. A free TAT manual is available for download at www.TATLife.com. A free EFT manual is available for download at www.emofree.com. Both these paradigms are easy to learn and can work wonders with unremitting pain.
