Dear Dr. Halevi,
My name is Dr. Eric Waltemate, I live in Missouri. I have a pt who came in the other day who has an ulcer on his left patellar tendon distal to the patella. He was in a wreck 17 years ago and had his leg completely severed from his thigh. The leg was reattatched but he has had this ulcer ever since. He has extensive scarring around the whole knee area and I was wondering what points to needle to get rid of that ulcer. He came in today and the ulcer had dried up after one treatment, but that could be because I told him to keep in uncovered. The other thing is that his eyes have a look of murderous rage in them. He looks like he could kill at any moment. His pulse was very wiry and he says he is aggravated at his condition and has no patience during the tratment. His needle sensation comes on very easily and he jerks before I can get the needle to the desired depth. (I use a 32 guage needle mostly unless I'm going to needle a very thin area of skin.)
Any help you could provide would be most appreciated.

Greetings Eric. Looks like a tough case, isn't it? It seems that you have a case of extreme Fire & Dampness + blood stasis at the knee region. I suggest that, especially in view of the fact that your pt is short tempered and impatient, you use only one point. Try blood letting on B-40 Weizhong after a swift insertion, Deqi and withdrawal of the needle. After the blood letting (a few scartches with a triangle needle), lay a cup over the point to squeeze some 2 cm2 of blood. This procedure has an excellent blood circulating effect over the whole knee. Repeat this procedure several times (every other day) until the patient feels much better. This will probably increase his confidence in your treatment so that you will be able to enlarge the treatment scale and treat him more comprehensively.
